123ContactForm attains HIPAA Compliant Status

123ContactForm announced that it has achieved
compliance with data security standards outlined in the Health Insurance Portability
and Accountability Act (HIPAA).

The decision was based on 123ContactForm’s customers feedback and on the increased
interest in medical forms such as Appointment Request, Medical History Form,
Prescription Refill, Online Bill Pay, Patient Satisfaction Survey and Dental History
Form. The progressive complexity of the forms was soon followed by a need to ask for
the patient’s private health information.

In order to fulfill these needs, the company has decided to become HIPAA compliant
by implementing specific demands: a customized Business Associate Agreement, HIPAA
training sessions for the employees of the covered entity, confidentiality
agreements signed by all the consultants who work for customers’ account
maintenance, 123ContactForm’s HIPAA Compliance Guide Sheet and strong limitations
regarding the way private information is being handled.

The Health Insurance Portability and Accountability Act of 1996 (HIPAA) is a federal
law designed to protect a subset of Sensitive Information known as protected health
information: Patient names, Telephone numbers, Fax numbers, Social Security numbers,
Full face photographs, Healthcare record numbers, Account numbers, Biometric
identifiers, Device identifiers, Health plan beneficiary numbers,
Certificate/license numbers or any other unique number, code, or characteristic that
can be linked to an individual. Certain organizations called “covered entities”
and their business associates are required to comply with HIPAA.
